Return Authorization Form

Please complete this form to request a return authorization number (RA#). Upon receipt and review by Scaletron, a return authorization number will be emailed to you along with a copy of your filled out form.

All components must be shipped prepaid via UPS or FEDEX (Do not use USPS!) to Scaletron Industries Ltd’s. factory authorized repair center. A copy of the completed RA Authorization Form that includes a written explanation of the apparent problem must accompany all returned components. ***Please No Packing Peanuts***

**IMPORTANT: Please note that once we have provided a quote for repairs we need approval within 30 days. After that time, the equipment will be scrapped.**
